These are the chord changes for Woody Shaw's most excellent composition The Moontrane. It first appeared on Larry Young's album "Unity" where Woody was a sideman,  then on Woody's album on Muse Records The Moontrane and finally on Dexter Gordon's "Sophisticated Giant" with Woody playing in a large ensemble. All these versions of the tune are great.
